Thank you for choosing Glam,
etc. To book your wedding date with Glam, etc. and secure our
services for your wedding day, we ask that you please fill out the requested information on this
form.
After you
have completed this form please click Submit, you will then be
directed to a new page where you will find a link to pay your
deposit online or an address to submit your deposit by check.
The
deposit amount to secure our services for your wedding date is $125. You will be provided a confirmation of payment and will
receive a receipt of payment from Glam, etc. and confirmation
that your wedding date is secured. Please submit complete
contact information so that we may provide you a receipt
promptly and are able to get in touch with you concerning your
wedding plans. Glam, etc. accepts personal checks, cashier check and money
orders. All security deposits are applied toward
the total cost of service and are 100% refundable if services
are
canceled at least 60 days prior to the scheduled wedding day.
